This facility does not currently meet the minimum staffing ratios required by law. Posted at the direction of the Illinois Department of Public Health. The period in question occurred during the fourth quarter of 2024. This staffing discrepancy was identified and promptly corrected followed by the development of a plan of correction to avoid future discrepancies. The Illinois Department of Public Health is currently reviewing our plan of correction.
